Department of Health fees are $50.00 for Mississippi artists and $100 for out-of-state artists.
Mail money orders payable to the Mississippi Department of Health with this Health Department Form, Current Government-Issued ID, Current Tattoo License, and Current Blood Borne Pathogen Certification to:
